Good afternoon folks,
I'm building my second 3D printer and my first Voron -- a V0.2 from the Formbot kit, using the Dragon HF hotend. So far the build has gone quite smoothly, but I've run into a snag the first time I heated it up -- it was jamming in odd ways, and eventually no filament would move through in either direction even when it was up to temp. I disassembled it and discovered that the hotend had melted the little plastic mounting adapter / shim thing, leaving the hotend itself able to wobble out of place.

This caused the PTFE tube to come disconnected from the hotend (since it only goes in by about 1mm), and allowed hot plastic to squidge out between the hotend and the adapter. See attached photos for details.

I have a few questions:
1. What mistakes in assembly or operation could have caused this? (I noticed the cooling fans weren't running -- could that be part of it?)
2. Do I need to put some kind of insulation between the hotend and the mounting adapter to prevent this from happening again?
3. This mounting adapter is ruined so I'm going to have to print a new one at my local makerspace. What considerations should I keep in mind while doing this?
4. Do I need to take any particular steps to clean out the hotend before reassembling the device?
